What is a window period?
The window period is the time between infection and when a test can reliably detect it. It varies by infection type and by test method. During the window period, you can be infectious even with a negative test result.
A negative test does not rule out risk if you are inside a window period. Repeat testing may be needed after exposure, and testing timing should account for the specific infection’s window.

Special considerations
Trans and nonbinary individuals

- Hormone therapy does not change testing frequency.
Sex workers and high-volume partners
- Consider monthly testing, more frequent than the standard quarterly cadence.
- Discuss with a provider what’s realistic and sustainable.
- Testing is care, not punishment.
After treatment
- Most conditions: retest 2-4 weeks post-treatment to confirm cure.
- Syphilis: serologic titers are monitored to confirm a declining response.
- Don’t retest the same day as treatment; false positives are possible.
Condom use and testing
- Condoms significantly reduce STI risk but don’t eliminate it.
- Combining condoms with testing is the most protective strategy.
- Condom use alone does not remove the need for regular testing.
Vaccination as prevention
- Hepatitis B: vaccinate instead of repeatedly testing.
- HPV: prevention through vaccination rather than detection.
- Meningitis is not sexually transmitted but may be recommended in some settings.

Test method reference
NAAT
- Best sensitivity and specificity for chlamydia, gonorrhea, and trichomonas.
- Can test urine or swabs (urethral, cervical, pharyngeal, rectal).
- Considered the gold standard.
Rapid tests
- Results in minutes.
- Lower sensitivity than NAAT; high specificity.
- Good for point-of-care; confirmatory testing often needed.
Serology (antibody tests)
- Detects immune response, not active infection.
- Used for HIV, syphilis, and Hepatitis B.
- Window period applies; antibodies take time to develop.
PCR
- Highly sensitive for herpes on active lesions.
- Less commonly used for other STIs than NAAT now.
Culture
- Gold standard for gonorrhea at pharyngeal or rectal sites, where NAAT is less sensitive.
- Slower, but useful for suspected antibiotic resistance.
- Not every clinic or lab offers every method listed here.
- Ask which method your provider is using and what it does and doesn’t rule out.
